The adjustable belt goes on the inside with the short one by the door on both seats. And, yes, the belt by the door flops out. I have just learned to hang on to it when undoing the belt and setting it down inside the car. The door sills take a beating with that soft aluminum still material.

You can get seat belt retractors but be careful as they snap back fast and hit the door!

My rockers and lower quarter panels are all chipped up in front of the rear tires due to the PO probably driving with the seatbelt hanging out the closed door. For that reason, I have reversed the belt's positions so that the buckle is closest to the door. It is longer, and I lay it by my feet when I unlatch it. My sons drive my car, and I cannot be certain that seatbelts are always worn.
Those fancy SD belts may end up with inverted ponies, if my strategy were followed.

Probably the best strategy is to stuff it down in the floor pan area every time you unbuckle the seat belt before getting out of the car. That's what I used to do before I went to 3-point belts.
I've seen the seat belt "stalks" (like the sleeved belts you're talking about) sold for our cars, but that part usually goes on the transmission tunnel side. If you had them on the outside I think you'd piss yourself off real fast trying to climb past the stiff upraised seat belt as you're trying to exit/enter the car.
