Does the MAF have to be located before the throttle body?
To me, it would measure the same amount of air being consumed by the engine.

Yes, it does. That's where its located in every OE application. I get the idea that air drawn across the sensor is air drawn across the sensor, but behind the throttle blades is an area of turbulent air, and would make accurate readings impossible.

Even clocking can disrupt proper air metering in the MAF. Ford also used a screen on the intake side of the MAF to help evenly distribute the air flow on some applications. I've talked to folks who report just clocking the MAF 90 degrees made significant improvement in idle or other parameters.

Is there a minimum length the intake assembly has to be?

I don't think its critical. That said I wouldn't put the MAF right on the end of the TB. It wasn't uncommon to move the MAF with some of the 5.0 intake setups. It was always right behind the airbox/air intake though, so I think I'd keep it in that location.

Only thing I ran across was to NOT place before a 90 deg elbow.

I'd give it some length of tube before the throttle body, I think it might give some weird readings directly on it.

MAF needs to be in front of the air intake for the the PCV, so that all the air going through the engine is metered.

Interesting, because most CAIs have a 90 coming right off the TB. The MAF is a good ways downstream of the 90, but there's a 90 there nonetheless.
